• Electronic Visa Application Form (DS-156).  Applicants for all types of non-immigrant visas are required to complete the application electronically.  The form can be accessed at http://evisaforms.state.gov.  Print all three pages, including the last page, which contains your barcoded information.
  
• Printable version of the Supplemental Non-immigrant Visa Application (Form DS-157).  Supplemental NIV application for all Turkish men between ages 16 and 45 and all male and female third-country applicants.
  
• Printable version of the Contact Information and Work History for Non-immigrant Visa Application (Form DS-158).  Supplemental NIV application for all F, M, and J visa applicants (and their dependents).
